What most patients search for as "vaginal PRP" is actually performed here using PRF — platelet-rich fibrin — which represents a meaningful clinical advancement over standard PRP. We made the switch from PRP to PRF over 5 years ago. The difference is not marketing. PRF eliminates the anticoagulant additives used in traditional PRP processing, which allows platelets to form a three-dimensional fibrin scaffold at the injection site. That scaffold holds 8–10 times the growth factor concentration of conventional PRP and releases those growth factors gradually over 7 to 15 days, rather than dissipating within hours. The result is a stronger, longer-lasting regenerative response in the treated tissue.

How Vaginal PRP and PRF Actually Works

The science behind vaginal PRF injections is the same regenerative biology that orthopedic surgeons and oral surgeons have relied on for over two decades — and that we use for skin rejuvenation and promoting hair growth — adapted for gynecological tissue.

Here is what happens at the cellular level: a small blood draw is processed in a specialized centrifuge without any chemical anticoagulants. The resulting PRF concentrate contains a dense mesh of fibrin, platelets, white blood cells, and stem cell cytokines — essentially a biological scaffold your body recognizes as its own.

Once injected, that scaffold does three things:

  1. Recruits stem cells to the injection site through platelet-derived growth factor (PDGF) and vascular endothelial growth factor (VEGF) signaling
  2. Stimulates new collagen synthesis, which increases tissue thickness and elasticity
  3. Promotes neovascularization — the growth of new blood vessels — which improves sensitivity and hydration in the treated area

A 2019 study published in the Journal of Cosmetic Dermatology found that PRP injections in vaginal tissue produced statistically significant improvements in mucosal thickness and self-reported sexual satisfaction at 12 weeks post-treatment. The PRF protocol used at The Dermatology and Laser Group builds on these findings with a higher growth factor payload and a sustained-release delivery mechanism.

"When I explain this procedure to patients, I compare it to fertilizing a garden," says Arash Akhavan, MD. "You are not adding anything artificial. The fibrin scaffold is what makes PRF different — it holds those growth factors in place long enough for real structural change to occur."

We recommend a series of four treatments, one month apart. Results typically last approximately 12 months, and many patients return for maintenance treatments more frequently to sustain their results.

A note on terminology: This procedure is sometimes marketed at other practices as the "O-Shot" — a branded term that Dr. Arash Akhavan does not use because it trivializes what is a genuine medical procedure backed by real regenerative science. The clinical technique is the same concept, but our practice uses PRF rather than standard PRP and every injection is performed by a board-certified dermatologist.

Three Anatomical Targets, Three Different Goals

Vaginal PRF is not a single procedure — it is a framework for treating three anatomically distinct areas, each with its own clinical rationale. Arash Akhavan, MD, FAAD, tailors the injection protocol to each patient’s specific concerns during the consultation.

G-Spot PRF Injection. The G-Spot — a region of dense nerve endings on the anterior vaginal wall approximately 2–3 centimeters inside the vaginal opening — responds to PRF by thickening and becoming more sensitive. Patients who report difficulty with vaginal orgasm or reduced sensation during intercourse are often candidates for this injection. The PRF is placed using a small-gauge needle with topical and local anesthesia, and the entire injection takes under five minutes.

Clitoral PRF Injection. The clitoris contains over 8,000 nerve endings, more than any other structure in the human body. PRF injections into the clitoral complex stimulate neovascularization and tissue regeneration in this highly innervated area. Patients seeking enhanced clitoral sensitivity or improved arousal response are candidates for this injection site.

Labia Majora PRF Injection. Volume loss in the labia majora is an extremely common concern — particularly after childbirth, weight fluctuation, or with natural aging. PRF injections into the labia majora restore fullness and improve skin quality from the inside out, without synthetic fillers. This approach, sometimes called the Vampire Wing Lift by other practices, uses the same regenerative PRF concentrate to plump deflated labial tissue and improve skin texture. The results look and feel natural because the volume comes from your body’s own collagen production, not an injectable gel.

Some patients choose a single target area. Others combine two or all three in the same visit. There is no additional downtime when combining injection sites.

What Patients Experience Before, During, and After Treatment

Before Your Appointment

No special preparation is required. Patients should be well-hydrated on the day of treatment, as hydration improves the quality of the blood draw and the resulting PRF concentrate.  There is no need to fast, stop other medications, or arrange for transportation home afterward.

During the Procedure

A topical numbing cream is applied to the treatment area approximately 20 minutes before injection. Pro-Nox (a self-administered blend of nitrous oxide and oxygen) is also always available if desired for all of our procedures to ensure your comfort throughout. A small blood draw is taken from the arm — the same process as a standard lab test. The blood is processed in a centrifuge for approximately 10 minutes to separate the PRF concentrate. Dr. Arash Akhavan then injects the PRF into the target area or areas using a fine-gauge needle. The injection itself takes less than five minutes per site. Total appointment time is approximately 30 minutes, including numbing and processing.

After Treatment

Patients leave the office immediately and can return to normal daily activities the same day. Mild swelling or tenderness at the injection site may occur for 24–48 hours. This is a normal part of the regenerative response. Sexual activity can typically be resumed within 24–48 hours, though Dr. Akhavan provides individualized aftercare instructions during the appointment. Initial improvements in sensitivity and tissue quality appear within 3–4 weeks. Full collagen remodeling results develop over 2–3 months.

Vaginal PRP Cost & Treatment Packages

A single vaginal PRF injection session is $850. For patients who want a full treatment course — which Dr. Arash Akhavan recommends for optimal collagen remodeling and sustained results — a package of four sessions is available for $3,060 (a 10% savings over individual pricing). Sessions in a package are typically spaced four to six weeks apart.

These prices apply regardless of whether the patient is treating one anatomical area (G-Spot, clitoris, or labia majora) or combining multiple sites in the same visit. There is no upcharge for treating additional areas during the same session.

What is the difference between vaginal PRP and vaginal PRF?

PRP (platelet-rich plasma) and PRF (platelet-rich fibrin) both use your own blood to stimulate tissue regeneration, but PRF is a second-generation advancement. The key difference is processing: PRP uses chemical anticoagulants that prevent clotting but also limit growth factor retention, while PRF is processed without additives, allowing platelets to form a natural fibrin scaffold. That scaffold concentrates growth factors at 8–10 times the level of standard PRP and releases them gradually over 7–15 days rather than hours. Does vaginal PRP hurt?

Is this the same as the O-Shot?

The procedure is sometimes marketed elsewhere as the “O-Shot,” a branded term Dr. Akhavan does not use because it trivializes what is a genuine medical procedure with real regenerative science behind it. The clinical technique — injecting autologous platelet concentrate into vaginal and vulvar tissue — is the same concept, but The Dermatology and Laser Group uses PRF rather than standard PRP, which offers a significantly higher growth factor concentration and sustained release. More importantly, every injection here is performed by a board-certified dermatologist, not delegated to non-physician staff.

Who is NOT a good candidate for vaginal PRF injections?

This procedure is not recommended for patients who are currently pregnant, have an active infection in the treatment area (such as an active herpes outbreak or bacterial vaginosis), have a bleeding disorder or are on anticoagulant therapy that cannot be paused, or have a platelet disorder that would compromise the quality of the PRF concentrate. Patients with a history of gynecological cancer should consult their oncologist before proceeding. Dr. Akhavan evaluates candidacy on a case-by-case basis during the consultation.

Can vaginal PRP be combined with other treatments?

Yes. Patients frequently combine vaginal PRF injections with other treatments offered at The Dermatology and Laser Group. ThermiVa — a radiofrequency-based vaginal tightening treatment — is the most common complement, as it addresses external tissue laxity while PRF addresses internal regeneration and sensitivity. Some patients also combine the Vampire Wing Lift (labia majora PRF) with labial puff hyaluronic acid filler for additional volume. How many vaginal PRP treatments do I need?
