Exosomes therapy is an exciting procedure in the field of regenerative aesthetics. Exosomes are tiny vesicles (sacks) that are produced by almost all cell types in our body and are used for communication between cells. Exosomes contain an abundance of growth factors and other proteins and molecules. For in office exosome treatments, the exosomes are isolated from cell cultures in a lab and injected into patients to signal rejuvenation, and to augment the body's own natural regenerative powers. The cell cultures used in the production of exosomes can be manipulated so that their composition of growth factors is optimized for their specific areas of use. For example, specific exosome sources might be produced that are better for use on the skin and soft tissue, while other exosome products may be produced specifically for hair growth.

At our office we use exosome products from AnteAGE. These highly concentrated solutions contain 10 billion exosomes that are filled with Growth Factors and anti-inlammatory cytokines that support healthy and more youthful skin. The exosomes are engineered from a culture of hybrid bone marrow stem cells (BMSC) + Wharton's Jelly cells (umbilical stem cells). In order to maintain the highest quality and purity the exosomes are lyophilized and then reconstituted immediately prior to injection.
Exosomes vs. PRP
The concept behind exosome therapy and PRP (platelet-rich plasma) treatment is the same. With both products we introducing a solution with concentrated growth factors to kickstart the body's natural rejuvenation pathways. In the case of PRP, the growth factors are concentrated from the patient's own blood. In the case of exosomes, the growth factors are produced by cells cultured in a lab. The advantages of exosomes over PRP include a higher concentration of growth factors and a consistently better composition of growth factors and anti-inflammatory proteins. The ingredients in exosomes have been optimized for tissue regeneration in the lab. Circulating proteins in PRP not only diminish with age, but can also have variable composition depending on a person's current health, nutrition, stress, and other factors.

Exosome Skin Rejuvenation
As we age, there is a significant decline in our body's ability to heal from injuries and generate collagen and elastin in tissue regeneration. Exosomes are an excellent way to provide a boost of growth factors to the skin and underlying tissues to promote rejuvenation, and increase collagen and elastin production. Use of exosomes becomes more important as we age into our 30s, 40s, and beyond as our natural growth factors diminish.
At our office we introduce exosomes into the skin with either injections using a tiny needle (similar to what is used for Botox injections) or during microneedling procedures. Applying them topically to the skin is useless, in our opinion, and will not lead to significant penetration of viable exosomes to the necessary depth.
Exosomes can be injected on their own as a stand-alone procedure, injected into the skin to improve smoothness and increase elasticity. We commonly use exosome injections on the face, neck, breasts, hands, thighs, and vulva. Exosome injections in the lips are also a great natural alternative to lip fillers.
Alternatively, exosome therapy can be used to compliment and add growth factors to other procedures. We often combine exosomes with with medical microneedling, where the microneedles can introduce the exosomes to the proper depth to be effective. We also use exosomes in conjunction with radiofrequency and fractional laser resurfacing procedures. When used with these procedures, it is most useful to inject the exosomes into the areas being treated with an needle.
Exosome Hair Therapy
Exosome injections for the hair are a great alternative to PRP scalp injections for patients looking for more robust and consistent results. PRP injections supporting hair growth are one of the more common procedures we have performed for the last decade at The Dermatology and Laser Group. As our technique for isolating, preparing, and injecting PRP has improved so have our results. But, like all practices that perform PRP scalp treatment, we sometimes have patients who respond poorly to the treatment, even when the right patients are selected and the correct technique is used. Unfortunately, not all patients have a great growth factor and protein composition circulating in their blood to support hair growth, and growth factors decline significantly past our 20s.
Exosome hair therapy injections are a great alternative that can consistently provide a robust and optimized solution of growth factors into the scalp.
