Does Tattoo Removal Leave Scars?
Medically Reviewed by Arash Akhavan, MD, FAAD
For many people, a tattoo that once held meaning can become something they no longer identify with. Whether it’s due to a shift in personal style, a career change, or simply outgrowing a past decision, the desire to remove an unwanted tattoo is incredibly common. In fact, tattoo removal is one of the fastest-growing cosmetic procedures in the U.S.
Patients who come into our office for laser tattoo removal typically want the ink gone as thoroughly and completely as possible. They’re looking for a clean slate -not just a faded shadow of the original tattoo- and they want to achieve those results with minimal damage to their skin. But as more people consider laser tattoo removal, one of the most frequently asked questions is:
Does tattoo removal leave scars?
PicoWay Lasers Are Less Likely to Cause Scarring
When it comes to removing tattoos with minimal risk to your skin, the type of laser used makes a significant difference. At The Dermatology and Laser Group, we exclusively use the PicoWay® picosecond laser, one of the most advanced laser technologies available today for tattoo removal.
Unlike traditional Q-switched lasers that rely on longer pulses of heat to break up tattoo pigment, PicoWay uses ultra-short bursts of energy - each lasting just a trillionth of a second. This rapid-fire delivery allows the laser energy to shatter tattoo ink into tiny particles without generating excessive heat in the surrounding tissue. This is important because less heat means less risk of burning, scabbing, and ultimately, scarring.
PicoWay also delivers better results in fewer sessions compared to older technology. It is highly effective at treating even difficult-to-remove colors like green and blue, and it can be used safely on a wide range of skin tones -something not all lasers can do. Its precision targeting means your surrounding skin remains largely unaffected, allowing for quicker healing and a much lower chance of long-term textural changes or discoloration.
The safety profile of the PicoWay laser, combined with our expertise in customizing each treatment to the patient’s unique skin type, tattoo characteristics, and healing response, gives patients the best possible chance at complete removal with minimal risk of visible skin changes.
Unfortunately, Tattoo Removal Can Sometimes Cause Scarring
Even with state-of-the-art technology like PicoWay, tattoo removal still carries some risk of scarring. This is an important part of the conversation we have with every patient who visits our NYC office for tattoo removal. Being realistic about the potential risks allows us to take extra precautions and set clear expectations for the outcome.
There are several factors that can influence the likelihood of scarring:
1. Tattoo Ink Characteristics
-
Density and saturation: Tattoos with dense or layered pigment may require more sessions or higher energy levels to fully break down the ink, which increases the chance of trauma to the skin.
-
Color of the ink: Some pigments such as red ink, especially when on darker skin tones, are more likely to lead to hypopigmentation and scarring.
-
Depth of ink placement: Tattoos placed too deeply into the dermis can be harder to fully remove, requiring greater precision and more treatments that can stress the skin.
2. Individual Skin Factors
-
Skin tone: Patients with darker skin are at higher risk of post-inflammatory hyperpigmentation (PIH), hypopigmentation, or scarring. Choosing a laser like PicoWay- which is safer for darker skin tones- is critical, but expertise in selecting the right settings is just as important.
-
Skin sensitivity and healing response: Some individuals simply scar more easily, even from minor trauma. A history of keloid or hypertrophic scar formation can raise your risk.
-
Previous tattoo complications: Tattoos that were infected, poorly healed, or already scarred are more likely to respond unpredictably to laser treatment.
3. Aftercare and Healing
-
Following post-treatment instructions is essential to minimizing complications. Scarring can occur if a patient picks at scabs, exposes the treated area to the sun too early, or doesn’t keep the site clean.
-
We provide each patient with detailed tattoo removal aftercare instructions and close follow-up to ensure the skin heals as smoothly as possible between sessions.
